Pflege-Dokumentation: So dokumentieren Sie richtig

Redaktion pfleg.ch Aktualisiert: 20. März 2026

Careful care documentation is important for family caregivers, both for communication with doctors and Spitex and for billing your compensation. In this guide, you will learn why documentation is important, what you should record and how to keep the effort low.

Why documentation is important

Care documentation serves multiple purposes: it documents the services provided for billing with the health insurer, enables a seamless handover to other care providers or Spitex and creates legal certainty in case of enquiries or audits.

If you are employed through a Spitex organisation and receive compensation, documentation is often mandatory. The Spitex and the health insurer require proof of the type, scope and timing of the care services provided. Without documentation, billing may be refused.

Documentation also helps when applying for the helplessness allowance or when justifying care levels. Doctors and authorities can get a clear picture of the care needs.

What you should document

As a general rule, you should record the following points:

  • Date and time of the care service
  • Type of service (e.g. personal hygiene, meal assistance, mobilisation)
  • Duration in minutes
  • Special incidents (e.g. fall, confusion, pain)

Keep entries short and factual. It is not about detailed reports, but about traceable facts. Many Spitex organisations provide templates or forms. Use these to minimise the effort.

Tips for daily documentation

To prevent documentation from becoming a burden, the following practices have proven effective:

  • Record immediately after care: Enter the service right away so you do not forget anything and the effort stays low.
  • Schedule fixed times: Reserve e.g. 5 minutes in the evening for daily entries.
  • Use digital tools: Apps or spreadsheets on your smartphone make documentation easier on the go.
  • Coordinate with Spitex: Ask which format is preferred to avoid duplicate work.

Care documentation does not have to be perfect. What matters more is that it is kept regularly and traceably. If you have questions, your Spitex organisation will support you, or if you are employed through pfleg.ch ag, our team.
